Referral to the Brisbane South Aged Care Assessment Organisation is via My Aged Care, ph 1800 200 422 or web-based referral www.myagedcare.gov.au The Aged Care Assessment Program (ACAP) is jointly funded by the Commonwealth Government and the States and Territories. Aged Care Assessment Organisations (formerly know as ACATs) are required to conduct a holistic assessment of the clients care needs - taking into account the physical, medical, psychiatric and social needs of frail older people, including their rehabilitation potential - in order to help them choose the most appropriate services to meet their needs. Assessment and approval by an Aged Care Assessment Organisation is mandatory for admission to residential care and residential respite care, and to receive assistance from Support at Home Packages or short term options such as Transition Care (inpatients only), Restorative Care or End-of-Life pathway. Assessment for low level community care is also offered (Home Support assessment).

Eligibility

Referral required. Appointment required. Waiting list.
The primary client group for ACATs are elderly people, and who have considerable difficulty functioning in their usual environment, and may require a package of services to continue to live in the community and without which would be at risk of premature or inappropriate long term residential care; or who are unable to remain at home and now require residential care. Younger people with disabilities are able to be assessed by the ACAT after all other appropriate care options aimed at younger people have been exhausted.
