Hospital Outpatients
The HFSS are multidisciplinary teams that assist patients with heart failure (HF) to adhere to treatment and manage symptoms. These teams are comprised of nurses with specialist medical support. Some services may include pharmacists, physiotherapists or exercise physiologists, and other allied health professionals. Services identify patients admitted to hospital with HF as well as taking outpatient referrals. Statewide coordination of these support services provides training and promotes an evidence-based, consistent approach in delivery of care across the state.
Queensland Heart Failure Services have specialist heart failure nurses and some also have dedicated pharmacists, physiotherapists, exercise physiologists, occupational therapists, dietitians, social workers and psychologists. Services offered usually include some or all of the following:
1. Follow-up in the community: home visits, telephone, clinics, or telehealth
2. Medical follow-up including liaison with GPs
3. Patient and carer education and self-management strategies including: fluid monitoring, dietary advice, exercise program, medication review and social psychological support.
