The Sunshine Coast Health (SCH) Post-Operative Discharge Support Service (PODSS) is delivered by the Virtual Acute Care Service (VACS). SCH PODSS is a medically led, 7-day a week service - staffed by SMOs, NPs, NUM, CNC, CNs, RNs and AOs. SCH PODSS supports post-operative patients with low-acuity surgical concerns for up to 30 days post hospital discharge. SCH PODSS provides patients with timely virtual reviews - and in partnership with the Surgical and Critical Care Services, in-person reviews when clinically indicated (General Surgery, Urology and Orthopaedics). The PODSS team work closely with the patients GP and do not take over the role of usual GP care. Typically, the PODSS team support post-operative patients with concerns related to: • wounds, dressings • early infection • unmanaged pain • mediation management queries • post-op instructions • post-operative bowel and bladder dysfunction Inclusion Criteria • patient clinically stable and safe to be managed as an outpatient (Does not require Emergency Department level care) • post-operative patient, 30 days post-acute care discharge • patients call concern relates to index operation • surgery specialty is: General Surgery, Urology, Orthopaedics, Gynaecology and ENT Exclusion Criteria • General Surgery: paediatric surgery • Non-Medicare eligible
