The Chermside Community Health Centre provides a range of services for people living in the northern suburbs of Brisbane. The centre is located on the campus of The Prince Charles Hospital near the Hamilton Road entrance. Our services •Community Based Rehabilitation Team (CBRT) •Diabetes Service (adults only) •Post Acute Care Service (PACS) •Indigenous Health Services •Alcohol and Drug Service •Melaleuca Clinic (opiate replacement therapy) •Complex Chronic Disease Team (CCDT) We also offer the following mental health services: •Resource Team •Older Persons Mental Health Service (OPMH) •Community Mental Health Service (CCMHS) •Mobile Intensive Rehabilitation Team (MIRT) •Indigenous Mental Health Services
