If you're a senior, or aged under 65 and living with a disability, we can help you access a range of health and community care services including: Assessment of your health and care needs Getting to the doctor, dentist or other health appointments Nursing care Help around the home for those in accommodation Help with tenancy issues Training on budgeting, preparing meals and nutrition Assistance to form community networks including social and work related Referrals to drug & alcohol and mental health services Help managing diabetes or other health problems Self-care, including showering and dressing Our services are designed to help you improve your health and independence, guiding you towards using mainstream services. Referrals We accept referrals by you or by others including family, community services or health professionals. Call us on 1800 Ozcare (1800 692 273) or use our online referral form. Costs These services are provided at no charge as part of the Australian Government's Commonwealth Home Support Program (CHSP) and the Queensland Government's Community Care program.
