Hart 4000 provides assistance to obtain housing, advocacy, information and referral options and brokerage funds as part of a case management process. Hart 4000 is a Housing Information and Referral Hub which offers client lead term case management for clients who are homeless or at risk of homelessness. Hart offers a range of support services including assistance with housing applications, visiting legal and mental health clinics. Street Outreach Wednesday 6am to 9am.
