Aged care
The Aged Care Team support older people to lead healthy independent lives whilst staying connected to their community. We assist older frail people to gain access to the type of care appropriate to their needs including approval for services such as Home Care, Residential Care (permanent or respite) or Transition Care. Comprehensive assessments are provided in the community or hospital setting.
Brief interventions and programs are available to support Healthy Ageing, these include supporting access to Geriatric Services through telehealth and the OTAGO program - a falls prevention initiative.

The primary client group for ACATs are elderly people, and who have considerable difficulty functioning in their usual environment, and may require a package of services to continue to live in the community and without which would be at risk of premature or inappropriate long term residential care; or who are unable to remain at home and now require residential care. Younger people with disabilities are able to be assessed by the ACAT after all other appropriate care options aimed at younger people have been exhausted.
