Chronic Disease management
program is offered for two hours, twice per week for eight weeks to better manage a chronic respiratory condition and improve your quality of life.
Respiratory Services offer domiciliary (home visit), community health centre and Redland and Logan Hospital based clinic-based services for people with respiratory conditions. Respiratory Service can provide information on client's current health condition to ensure better understanding and assist in development of self management techniques. as well as help you coordinate your care
