***The SMoCC service is currently not available as it is being redeveloped to focus on persons living with diabetes and enhance virtual care delivery features. It is expected that the service will reopen in the latter half of 2026.***
The SMoCC service, delivers The COACH Program which is an evidence-based, state-wide, telephone-delivered chronic conditions program that supports consumers with:
• Coronary Artery Disease
• Type 2 Diabetes
• Pre-Diabetes or
• COPD (emphysema or chronic bronchitis)
The service employs a patient-centred approach to goal-setting and self-management that assists, motivates and enables participants to change the behaviours known to contribute to their chronic conditions. Consumers enrolled on The COACH Program will receive a phone call with a Registered Nurse every 4-6 weeks for a period of around 6 months.
Patients are coached to achieve national guideline recommended target levels for their particular chronic condition, which can improve their health and reduce avoidable hospital admissions..

• Translator Service available
• Alternate form of delivery for cardiac rehabilitation
• Enables patients to better manage their health and symptoms of chronic disease

• Consumers can self-refer to the program by calling 13Health (13 43 25 84) and leaving a message for a nurse in the SMoCC service and they will receive a call back

Individuals must be over the age of 18, be self-caring and a resident of Queensland and have been diagnosed.
