Also known as GCPHN
Population Health, Way to Wellness Program
Primary Health Networks (PHNs) were established on July 1st 2015 with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time.
Our Strategic Goals are to:
Improve coordination of care to ensure patients receive the right care at the right place at the right time.
Increase efficiency and effectiveness of health services for patients particularly those at risk of poor outcomes.
Engage and support general practice and other stakeholders to facilitate improvements in our local health system.
Be a high performing, efficient and accountable organisation
The Gold Coast Primary Health Network (GCPHN) is working together with health care providers and the community to build a world class health system for the Gold Coast. We engage with, and support, general practice as the cornerstone of primary health care.
General practice is a highly valued partner and we continue to work together to improve services for our community. To do this, we need to better understand general practice in our region so that we can effectively engage with clinicians and staff.
