Home Medicine Reviews (HMR) are Home-based medication reviews that are conducted by accredited pharmacists. HMRs assist general practitioners and patients with suitable, appropriate use and management of medications. HMR can be conducted on an annual basis or more often if there are significant changes in a patients condition or medication regime. HMR's are fully subsidised by the Australian government.

GP's and community pharmacies are remunerated for conducting HMR by the federal government through the 4th community pharmacy agreement. Patients may be charged their usual consultation fees, however there is no cost to the patient for a HMR
