Bridges Reconnect is an early intervention program designed to support young people aged 12 to 18 who are at risk of homelessness, currently experiencing homelessness, or facing significant challenges in their lives. The program provides tailored case management to help young individuals reconnect with their families, communities, and opportunities for education and employment, empowering them to build a stable and successful future.

Eligibility

Referral required. No appointment required. No waiting list. Self / family / friends
Online referral: https://intake.lutheranservices.org.au/ ELIGIBILITY 12-18 year of age - homeless or at risk of homelessness, ongoing conflicts within family unit. Willingness to re engage with family, education, employment and community. Live or attend school within the Logan and South Brisbane region.

Female and male staff available
early Intervention program and case management for 3-6 months providing goal planning, brief counselling, family mediation, advocacy, providing information and soft referrals to appropriate services.
