Community Health and Navigation Support Services (CHNSS) is an independent nurse-led service providing healthcare navigation, clinical nursing consultation, care coordination and community-based support for adults, older people and people living with disability or chronic health conditions. We assist individuals and families to navigate complex health, aged care and disability systems through comprehensive nursing assessments, care planning, advocacy, service coordination, hospital discharge support, chronic disease management, continence and wound care advice, medication support, health education and referrals. CHNSS also partners with healthcare providers and community organisations to strengthen clinical governance, quality improvement and integrated models of care. We have a strong focus on culturally diverse communities, ensuring people receive coordinated, person-centred care that promotes independence, wellbeing and improved health outcomes.

CHNSS provides services to adults, older people and people living with disability or chronic health conditions. Services are available to private clients, NDIS participants, My Aged Care participants and healthcare providers seeking clinical consultation or organisational support.

Fees vary depending on the service provided. CHNSS accepts private clients, self-managed and plan-managed NDIS participants, and My Aged Care clients where applicable. Quotes are available for organisational consultancy services. Please contact us to discuss fees and funding options before your appointment.

CHNSS is committed to providing culturally safe, respectful and person-centred care for Aboriginal and Torres Strait Islander peoples. We work collaboratively with clients, families and local Aboriginal and Torres Strait Islander health services to support culturally appropriate care.
Services are provided in English and French, with experience working alongside accredited interpreters to support people from culturally and linguistically diverse (CALD) backgrounds. CHNSS has extensive experience supporting multicultural communities and can arrange interpreter services when required.
