Community Health services vary from centre to centre and include family health services, child health services, HACC, community health nursing services, confidential counselling, referrals to self-help groups and health welfare service, physiotherapy, occupational therapy, and Aboriginal and Torres Strait Islander services may also be available. Some Community Health Centres offer access to Home and Community Care (HACC) programs. HACC programs are a joint state and federal government initiative which provide frail aged people and people with a disability access to practical services which assist them to continue living in their own home. Support services are also available for carers. Services which may be available to eligible clients include domestic assistance, social support, respite care (in home and centre based), minor modifications after being assessed by occupational therapists, allied health services (such as dietetics, physiotherapy and speech pathology) and referral to other support services where needed (such as Meals on Wheels). Fees apply to certain HACC services, but special arrangements may be made for people who cannot afford these costs.
