If you are an inpatient, please speak with inpatient clinicians (Social Worker or discharge planner) regarding the process for inpatient assessments. If you are under 65 please contact My Aged Care regarding the process for younger people applying for aged care services. The Aged Care Assessment Program (ACAP) is jointly funded by the Commonwealth Government and the States and Territories. Aged Care Assessment Teams (ACATs) are required to conduct a holistic assessment of the clients care needs - taking into account the physical, medical, psychiatric and social needs of frail older people, including their rehabilitation potential - in order to help them choose the most appropriate services to meet their needs. Assessment and approval by an ACAT is mandatory for admission to residential care and residential respite care, to receive assistance from Home Care Package HCP or a range of flexible care options including Transition Care, Short Term Restorative Care (STRC) ALL REFERRALS NEED TO BE REGISTERED THROUGH MY AGED CARE.

The primary client group for ACATs are clients aged over 65 who have considerable difficulty functioning in their usual environment and may require a package of services to continue to live in the community and without which would be at risk of premature or inappropriate long term residential care; or who are unable to remain at home and now require residential care. Younger people with disabilities are only able to be assessed by the ACAT after all other appropriate care options aimed at younger people have been exhausted Referrals must come through My Aged Care. ALL REFERRALS MUST BE MADE BY CONTACTING MY AGED CARE 1800 200 422. ACAT does not accept direct referrals. Hospital Inpatients need to speak with inpatient clinicians (social workers or discharge planners) regarding referral process. Referrals are triaged according to Commonwealth criteria and clinical needs to determine urgency.
