The Aged Care Assessment Team (ACAT) is a multi-disciplinary team whose primary role is the assessment of frail aged people over 70 yrs and some people with disabilities, in their home, in hospital or in aged care facilities. ACAT is responsible for assessing eligibility and approving applications for Australian Government subsidised Residential care (Permanent and Respite) Home Care Packages and Transition Care (when a person is in hospital). The team provides information and advice to elderly people and some people with disabilities in order to assist them in making informed decisions about care services. Self referrals and referrals from any appropriate source are accepted when consent has been obtained from the elderly person.
